Program Offerings

· Nutrition Support Service - pharmacists work with other members of the healthcare team to create, implement and continually assess orders for our parenteral/enteral patients

· Infectious Disease Service - our pharmacists dose, adjust and monitor the patients on our ID service, working in close relation with ID physicians and other healthcare professionals

· Specialty Pharmacy Services - Vital Care pharmacists address various types of medication needs for our patients with a diverse group of disease states

· Transitional Care - pharmacists work directly with case managers to determine continuity of care plans for hospital inpatients upon discharge

· Pharmacy Compounding - pharmacists have a complete support system for preparing unique dosage forms in our innovative compounding pharmacy
